Physician referral is not required by Medicare — your patient may self-enroll at areyouatriskfortype2.com. A referral from you ensures we have clinical documentation and allows us to send you progress updates.

Direct Patient Instruction

Tell your patient: "Go to www.areyouatriskfortype2.com and click enroll now. Your Medicare a Medicare covers this program at no cost."


Submit a Referral

Fill out the form. We handle the rest.

Once you submit a referral, TNN contacts your patient within 1 business day, verifies Medicare eligibility, and enrolls them in the next available cohort. With your patient's written authorization, we'll send you attendance and milestone updates throughout the program.
