The National DPP lifestyle change program is one of the most rigorously studied behavioral interventions in preventive medicine — reducing the risk of type 2 diabetes by 58% in people with prediabetes (71% for adults over 60). TNN delivers the CDC PreventT2 curriculum through live virtual group sessions and asynchronous distance learning, authorized under the PREVENT DIABETES Act through December 31, 2029.

22 Sessions Over 12 Months

16 weekly core sessions (months 1–6) followed by 6 monthly maintenance sessions (months 7–12). Patients are never dropped after the core curriculum ends.

Club 150 — Movement Your Way

Participant-led physical activity program targeting 150 minutes per week. Patients explore activity types and build a routine that fits their real life. No gym required.

Feeding Me — Basic Nutrition Education

10-session foundational nutrition education series. General nutrition education only — not medical nutrition therapy (MNT).

$0 Cost to Qualifying Medicare Part B Patients

TNN bills Medicare directly using HCPCS G-codes. No copay, no deductible, no cost sharing for qualifying beneficiaries. Self-pay options available for non-Medicare patients.

Gwendolyn Woody is the founder of The Nutritionist and Nurse and a nurse-trained diabetes lifestyle coach based in Houston, Texas. She built TNN specifically to deliver evidence-based diabetes prevention to Medicare beneficiaries with prediabetes — combining the clinical standards of a healthcare practice with the personal accountability of a coach.

Every session your patient attends is delivered under Gwendolyn's clinical supervision. Weight is documented every session. Progress is tracked against the CDC PreventT2 curriculum. And with your patient's written authorization, you will receive periodic updates on their attendance and weight loss milestones.

Once you submit a referral, TNN contacts your patient within 1 business day, verifies Medicare eligibility, and enrolls them in the next available cohort. With your patient's written authorization, we'll send you attendance and milestone updates throughout the program.
